DISABILITY CERTIFICATE
1. Till now Disability Cert to all citizens including ex-servicemen was being issued by the nominated Central Govt hospitals. After sustained efforts the case has finally been Govt of India, Ministry of Defense Gazette Notification No 3(4)/2009/D (Coord) dated 18 heb 2009 lays down the auth and the list of Armed Forces Hospitals, nominated for the issue of disability cert. A copy of Govt of India, Ministry of Defense Gazette Notification No 3(4)/2009/D(Coord) dated 18 Feb 2009 regarding issue of disability certificate to Armed Forces Service Personnel and their entitled dependents, Ex-Servicemen and their entitled dependents through the Armed Forces Hospitals is enclosed herewith for your necessary action.
3. The contents of Gazette Notification may please be disseminated to a formations/units under your command.

- Tlie process of issue of disability certificate, will be through medical boards constituted by an order from Commandant or Commanding Officer Armed Forces
- Percentage of disablement will be as per guidelines issued in the Notification in the Ministry of Social Justice and Empowerment vide No. 16~f 8/97-NLh published hi Gazette of India, part I Section h dated T June, 2001. The format of the certificate will be as per Annexure R ofthe said Notification.
- The certificate will be signed by Commandant or Officiating Commandant of service
- The medical board constituted for issue of disability certificate will be different from medical board constituted for Release Medical Board or Invaliding Medical Board purpose.

My self Ex AC 634953K Desai J. T. invalided Medically Borded out in 1982 , after more than 5years of service, since 21-02-1977 to 20-06-1982.
Boarded out in lower medical category “EEE”
When I joined I was perfectly fit.
I was treated in Military Hospital Jalandhar before medically Boarded out/discharged in 1982.
While Discharge Or after that no service Pension, or Medical pension or Alimony amount paid till today, not even given my medically boarded out papers too.
Only Service Discharhe Book given in that it’s mention as reason of Discharge….. Invalided Medical Board…… Authority…….. AFRO Signal No RRD/698 signed on 3-5-1982 in 108 Sqn AF c/o 56 APO by Wing commander K.Dey our Commanding Officer.
Once I recieved Invalide Graduty & DCRG an amount of ₹545/- from Air Force Central Account Office New Delhi on Dt 13-07-1989.
It’s my humble request to get me my alledical papers including Disability Certificate & all the Pensions for which a disabled soldier is eligible.
